Semaglutide Texas: results over the long haul

A large result deserves a long horizon. Weight, time and health outcomes in a field folio of the published trials.

Weight loss over time

Texas gives this folio its sense of distance. A dramatic result can register with me as a major milepost while leaving me impatient for the follow-up. How does the effect look when the horizon moves farther away?

Start with STEP 1: a mean body-weight change of −14.9% with semaglutide versus −2.4% with placebo at 68 weeks, in adults with overweight or obesity without diabetes. I call that substantial. Then turn to STEP 5, which found sustained weight reduction over 104 weeks in adults with overweight or obesity. Duration gives the claim another dimension.

The weight-loss folio puts those findings beside the direct comparison with tirzepatide. It also asks where the evidence runs out of road.

Cardiovascular and kidney trials

The paper that widens the whole subject is SELECT, a cardiovascular-outcomes trial in 17,604 adults with established cardiovascular disease and overweight or obesity, without diabetes. Semaglutide reduced the composite of cardiovascular death, nonfatal heart attack or nonfatal stroke versus placebo. That gets my attention for a different reason from a change on the scales: the measured outcome concerns serious health events.

FLOW studied major kidney-disease events in people with type 2 diabetes and chronic kidney disease. The heart and kidney folio follows those populations separately. I resist turning such specific findings into a general claim about everybody's future health.

Appetite research and alcohol

There is a smaller, stranger side road. Rodent experiments traced weight loss through distributed brain pathways; a human alcohol-use-disorder trial enrolled 48 adults and followed drinking outcomes over 9 weeks. The scale of that human experiment is plainly different from SELECT's.

I'm curious about this extension of the appetite story, with a much shorter leash on my enthusiasm. The appetite and alcohol folio follows the actual experiments without assigning them the reach of the larger trials.

Side effects in context

Benefits do not make the tolerability questions disappear. A semaglutide safety review describes gastrointestinal effects and increased biliary-disease risk, while SUSTAIN-6 recorded more diabetic-retinopathy complications in its high cardiovascular-risk diabetes population.

The side-effects folio gives those findings room. This site contains no local treatment directory: Texas is the editorial name, and the research discussed here makes no Texas-specific finding. The reading list and about page complete the folio.
